Midwest Eye-Banks

We were approached by Upshot to create some custom donation forms, for four websites associated with the Midwest Eye-Banks. These four sites (Midwest, Illinois, New Jersey and Michigan) all required the new multi-page form with a link to Authorize.net for payment.

Upshot provided the design and project management skills and it was purpleplanet's brief to create, implement and test the donation forms.
